A patient’s well being is monitored each visit and they are encouraged towards self-help, regaining confidence and maintaining independence. Nursing care is given as necessary eg: renewal of dressings and advice on symptom control.
Should the patient decide to attend on a regular basis, the clinical team will complete an assessment during which the patient will be encouraged to shared their needs and concerns. Following this, an individualised care plan will be formulated. The care the patients receive is reviewed regularly by the whole clinical team.
The opportunity to talk about advance care planning, patient preferences and wishes about the future care/end of life decisions will be offered.
Signposting to other services, e.g. Family Support or CAB is made if appropriate.
