Health professionals with specialist skills and experience in palliative care are responsible for your care whilst you are attending Day Hospice.
A Nurse Practitioner will make an initial assessment of the care that you may require. This assessment will take place at KEMP, a hospital or your home dependant on need. A member of your family will also be invited to attend. A Plan of Care will be discussed and implemented which will meet your individual needs. The care that you receive will be reviewed regularly by a Nurse Practitioner in partnership with you.
We aim to provide a seamless package of care ensuring close liaison with your GP, District Nurse, Macmillan nurse and hospital consultant as required.
At Kidderminster we provide care for up to 15 patients Monday, Tuesday, Wednesday and Friday.
Students
We participate in training programmes for students from a variety of courses/establishments eg; medical, occupational, physiotherapy students and work experience.
