Services at KEMP Hospice

We offer a range of support to patients and their loved ones who are dealing with a life-limiting illness.

An individualised plan of support is agreed with the patient/family which is reviewed regularly. This will be discussed at a KEMP multi-disciplinary team meeting. The KEMP MDT team works closely with health, social and allied professionals ensuring a seamless approach to care and support.

All patients are offered an initial holistic assessment. A KEMP Hospice Nurse will meet the patient, family & carer(s) to discuss how we can support them and agree a bespoke care plan. This can include:

  • Complementary Therapies
  • Group activities (creative therapies, social support and relaxation, breathlessness and exercise sessions)
  • Occupational Therapy – assessing patients’ needs to help them remain in their homes and maintain independence
  • Maintaining function through physiotherapy/exercise
  • Holistic ongoing nursing assessment, review of symptoms and needs through home visits, one to ones on site
  • Financial & welfare assessment, support, and review over the phone, at the hospice
  • Virtual support opportunities including group sessions
  • Hospice Nursing Service to support patients onsite and available to offer phone support and advice to patients, carers & professionals who call in
  • Carers Support planned programme of support which will be held onsite in small groups
  • Counselling & Bereavement Services provided by the Family Support to Family Support & Bereavement Service Team
  • Provision of hot & cold food for anyone visiting KEMP catering for all dietary & medical requirements
