Find details below of how to make a referral for KEMP Hospice Services.

All services at KEMP Hospice are available free of charge thanks to kind donations and the support of our local community.

Make a referral for KEMP Day Hospice services:

KEMP Hospice offers day services at the Hospice and also outreaching services into the community.

We support patients who are 18 years and over who have a palliative diagnosis of a life-limiting illness and are registered with a Wyre Forest GP.

Our team comprises Palliative Nursing Team, Occupational Therapy, Exercise and Well-being Services, Complementary therapy, Finance and Welfare Advice and Creative Arts.

Please return completed Day Hospice referral forms to KEMP Hospice by post or email.

KEMP Hospice Counselling Services

Our counselling and emotional support provides reassurance and a safe space for adults with, or affected by, a life-limiting illness or bereavement, and children and young people facing life without a loved one, to explore and express their emotions.

Make a referral for Adult Counselling Services

To make a referral for Adult Counselling services at KEMP Hospice you will need to:

  • be living in the Wyre Forest or registered with a Wyre Forest GP.
  • have experienced a bereavement in the last five years, or you have been impacted by a palliative diagnosis (either yourself or someone you know), or you are a KEMP Hospice Patient.

Referrals can be made by your health professional, or you can make a referral for yourself. Please download the form.

Make a referral for Children and Young People’s Counselling Services

To make a referral, the child and/or young person will need to:

  • be living in the Wyre Forest or attending a School located in the Wyre Forest.
  • have experienced a bereavement in the last five years.

£10 per month could pay for a Day Hospice patient to enjoy a creative therapy session, a welcome distraction from their illness and worries.

£52 could pay for a bereavement counselling session for a child or young person who is facing or living with the loss of a parent, grandparent or other loved one.
