What is KEMP Hospice at Home

Specialist palliative care

support from registered nurses and healthcare support workers who are trained and experienced to provide specialist end of life care.

Meeting the needs of the person

with a tailored programme of care and support based on the individual’s needs.

Familiarity of home

receiving care and support in the comfort of home and in familiar surroundings.

Comfort and dignity

during the final months, weeks and days of life with compassionate care and support.

Flexible care

reviewed and adjusted as the needs of the individual change.

Our service is designed to complement other services and professionals involved in the patient’s care. Depending on the individual’s needs and service capacity, we can provide the following:


Personal Care:

Assistance with daily activities such as bathing, dressing, and medication, while awaiting a care package or other support arrangements.


Respite Care:

Short-term care to provide family members or primary carers with the opportunity to rest and recharge.


Discharge Support:

Home-to-die support to help individuals settle back at home when discharged from hospital or inpatient hospice, particularly for those nearing the end of life.

Frequently Asked Questions

Have a question about our Hospice at Home service? We’ve answered the most common ones below.

To access the KEMP Hospice at Home service, patients must meet the following criteria:

  • End-of-life care needs: The individual must be in the last month, weeks, or days of life and wish to remain at home.
  • Short-term care needs: The individual must be awaiting Continuing Healthcare (CHC) fast track or awaiting an increased care package to be organised.
  • Capacity: Service provision is subject to availability and capacity.
